National Provider Identifier (NPI)

Effective May 23, 2007, Alameda Alliance for Health (Alliance) providers must use their National Provider Identifier (NPI) on all electronic submissions sent to the Alliance. Failure to report the NPI to the Alliance may cause a delay in claims reimbursement. Please follow the instructions below to comply with this requirement.

1. Apply for an NPI number. You may apply online at www.nppes.cms.hhs.gov or by mail. To request an application by mail, please contact the NPI Enumerator at 1-800-465-3203 or TTY 1-800-692-2326. To learn more about NPI, visit the Centers for Medicare & Medicaid Services (CMS) Web site at www.cms.hhs.gov/NationalProvIdentStand/.

2. Please obtain and submit your taxonomy specialty code with your NPI. Taxonomy information, including a complete list of taxonomy codes, is available at www.wpc-edi.com/taxonomy.

3. Report your NPI number and taxonomy specialty code to the Alliance by completing the Change of Provider Information Form online or by printing and filling out the Provider Information form and faxing it to Provider Services at (510) 747-4508.
