The Department of Health Care Services (DHCS) requires the Alliance network of primary care providers (PCPs) to ask Alliance members to complete the Staying Healthy Assessment (SHA) or approved alternate Individual Health Education Behavioral Assessment (IHEBA) within 120 days of enrollment and periodically thereafter.

SHA Forms
DHCS has SHA forms available for the following age groups:
- 0-6 months
- 1-2 years
- 3-4 years
- 9-11 years
- 12-17 years
- Adults and seniors
The assessments are available online in the following languages: English, Spanish, Chinese Vietnamese, Armenian, Hmong, Korean, Russian, and Tagalog.

Provider Training on the SHA
All Alliance Primary Care Providers must be trained on how to implement the SHA. If you have not yet completed the training, please do so as soon as possible.
