What is the Medication Therapy Management (MTM) program? 

The Medication Therapy Management (MTM) program is a free service offered to eligible Medicare Part D members. The program is designed to help members with complex health needs get the most benefit from their medications.  

If you qualify, a licensed pharmacist will: 

  • Review all your prescriptions and over-the-counter medications 
  • Talk with you about any side effects, drug interactions, or concerns 
  • Help you better understand how, when, and why to take your medications 
  • Answer any questions you may have 

What will you receive?

If you’re eligible for MTM services, you will be contacted to schedule a Comprehensive Medication Review (CMR) by phone. During this review, a pharmacist will go over your medications and help you create a plan to manage them safely and effectively. 

After your review, you will receive: 

  • A personal Medication List with the names of all your medications, how much you take, when you take them, and why you take them 
  • A Medication Action Plan with steps you can take to help you get the best results from your medication 
  • Educational information, including how to safely dispose of prescription drugs that are controlled substances 

You may also receive Target Medication Reviews (TMRs) throughout the year. These reviews check for any new medication-related issues and help make sure your medications continue to be safe and effective. You may be contacted by phone or mail if any concerns are found. 

How do I qualify and enroll?

You must meet all three of the following to be eligible for MTM:  

  1. Have specific chronic conditions.
    You must have at least three of the following core chronic illnesses:  
    • Alzheimer’s disease 
    • Bone disease-arthritis (including osteoporosis, osteoarthritis, and rheumatoid arthritis) 
    • Chronic congestive heart failure (CHF) 
    • Diabetes 
    • Dyslipidemia 
    • End-stage renal disease (ESRD) 
    • Human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS)
    • Hypertension
    • Mental health (including depression, schizophrenia, bipolar disorder, and other chronic/disabling mental health conditions)
    • Respiratory disease (including asthma, chronic obstructive pulmonary disease (COPD), and other chronic lung disorders) 
  1.   Have at least eight (8) maintenance/chronic Part D medications per month.
  2.   Must be likely to incur at least $1,276 in total Part D drug cost during 2026 (or estimated based on one fourth of that spending within the past three months). 

If you qualify, you will be automatically enrolled and contacted to participate. You do not need to sign up, and there is no cost to you.

Real Time Benefit Tool

We offer a real-time benefit comparison  (RTBC) tool so enrollees can obtain information about lower-cost alternative therapies under their prescription drug benefit plan. Enrollees can compare cost sharing to find the most cost-effective prescription drugs for their health needs. For example, if a doctor recommends a specific cholesterol-lowering drug, the enrollee could look up what the co-pay would be and see if a different, similarly effective option might save the enrollee money. With this tool, enrollees will be better able to know what they will need to pay before they are standing at the pharmacy counter. For more information about RTBC, please see below:
