What is a Dual-Eligible Special Needs (D‑SNP) Plan?
A D-SNP plan is a special type of Medicare Advantage Plan designed for people who qualify for both Medicare and Medi-Cal. These plans offer added support and extra benefits. Alameda Alliance Wellness brings your Medicare and Medi‑Cal coverage together in one coordinated plan that makes it easier to get the care, services, and support you need.
Who is eligible?
Dual EligibilityYou are enrolled in both full‑scope Medicaid (Medi-Cal) and Original Medicare (Parts A and B). |
Residency RequirementsYou must live in Alameda County. |
Enrollment PeriodDual-eligibles may enroll in a Dual Eligible Special Needs Plan (D-SNP) year-round. |

* Alliance Agents are available:
Monday – Sunday (seven (7) days a week), 8 am – 8 pm, from October 1 – March 31
Monday – Friday, 8 am – 5 pm, from April 1 – September 3
Alameda Alliance Wellness is an HMO D-SNP plan with a Medicare contract and a contract with the California State Medi-Cal (Medicaid) Program. Enrollment in Alameda Alliance Wellness depends on contract renewal.
