The Alliance offers the Health Homes Program (HHP) offers the Health Homes Program (HHP) for members with certain health care needs. HHP can help you get the health care and community support services you need.

When you join this program, you will have your own care team to support you as you work toward your health goals.

Who is Eligible

  • Alliance Medi-Cal members who have certain health conditions; and
  • Who have been in the hospital or emergency room; or
  • Without stable housing.

How to Enroll

  • You can work with your doctor to find out if you qualify for this program. Your doctor can refer you.
  • The Alliance may contact you to find out if you would like to enroll.
