Providers in the Alliance network must use their National Provider Identifier (NPI) on all electronic submissions sent to the Alliance. Failure to report the NPI to the Alliance may cause a delay in claims reimbursement.

Obtain and submit your taxonomy specialty code with your NPI.
Taxonomy information, including a complete list of taxonomy codes can be found through the Washington Publishing Company.
Report your NPI number and taxonomy specialty code to the Alliance.
Submit your NPI and other provider information on your office letterhead with the provider or office manager’s signature to the Alliance via fax to:
