Alameda Alliance for Health (Alliance) values our relationship with the provider community serving our members in hospital emergency departments. The Alliance wants to provide hospital emergency departments with resources to communicate with our team and members being served in your facilities to help ensure access and coordination.

How to reach key Alliance staff and departments:

  1. Contact information for coordination of emergency and Post-Stabilization Care
    The Alliance has a nurse responsible for coordinating Emergency Services, available twenty-four (24) hours a day to coordinate emergency and Post-Stabilization Care Services. The phone number is 1.510.747.4540 (toll-free 1.855.891.9161) during business hours. After business hours/nights/weekends, please call 1.510.326.5271. If the Alliance does not respond to a phone call from an ED for potential admission within thirty (30) minutes, the admission to the hospital is approved.
  2. Coordination of members’ care needs
    Care coordination is available for all post-ER or hospitalization needs through the member’s primary care provider (PCP) (found on their insurance card) and the Alliance Case and Disease Management program.   3. Communicating with the Alliance triage and Advice Systems
    The Alliance has a twenty-four (24) hour Nurse Advice Line for our members to receive advice on symptoms and healthy living, as well as when and how to access care. The phone number for our Medi-Cal members is 1.888.433.1876, and for our Group Care members is 1.855.383.7873.

  6. To report Alliance system or protocol failures, contact Provider Services at 1.510.747.4510 or email providerservices@alamedaalliance.org. The Alliance investigates all system/protocol failures and imposes corrective actions to remedy the failures.
  7. Authorization for medically necessary Post-Stabilization care is through the UM department. For admission requests, fax face sheets to 1.855.313.6306. Please fax clinical information to 1.855.891.7409.
