With Care Management, we provide nurses and care specialists to help you coordinate care, manage complex health needs, and understand your next steps.

If managing your health has started to feel like too much, we are here to walk with you, help you stay organized, and guide you through your care.

Every Alameda Alliance Wellness member is assigned a care coordinator when enrolling. Your care coordinator is a licensed nurse or social worker who helps you get all the health care services you need. They work with you to complete your Health Risk Assessment (HRA) and partner with your primary care provider to create an Integrated Care Plan (ICP) that keeps your health on track. They can also connect you with community resources and programs that support your health conditions or concerns.

Please complete your Health Risk Assessment (HRA) to help us understand your needs. Once completed, you will be assigned a care manager who will work with you to create a personalized care plan.

You can complete your HRA by mailing back the form or by calling the phone number on your member ID card.
